A key driver of the team's success has been making every patient interaction count. Whether in person or over the phone, staff take opportunities to update clinical information and ensure patient records remain current.
This progress has been supported by a whole-team approach, with nurses leading patient recalls and outreach, and Health Coaches, Navigators and Healthcare Assistants helping to connect with whānau and support follow-up care. Their proactive efforts have also contributed to improvements in smoking cessation and alcohol screening indicators.
The team attributes their success to clear communication, shared ownership and everyone working together towards better health outcomes for their community.
